眼球摘除术后植入未包裹的羟基磷灰石眼座的暴露率分析
【摘要】:目的分析眼球摘除术后一期植入未包裹的羟基磷灰石眼座的长期的暴露率及其可能的危险因素。方法对浙江大学医学院附属第二医院眼科中心2008年5月至2013年4月收治的234例两种不同手术技术进行的未包裹的羟基磷灰石眼座植入患者的病例资料进行回顾性病例分析,评价羟基磷灰石眼座植入后暴露率原因。结果234例病例中,151例进行了直肌止端端-端缝合术,83例进行了直肌原位缝合技术,随访时间从25个月到69个月(平均41.9个月)其中出现了 11例眼座暴露。其中3例出现在直肌止端端-端缝合组中(2%),8例出现在直肌原位缝合组中(9.6%),在直肌止端端-端缝合术组中,因为形成了复层直肌交叉结构,在眼座表面形成保护,所以眼座的暴露率降低了(OR=8.11,p=0.013),既往眼部手术被证实是一个增加眼座暴露率的因素(OR=2.73,p=0.032)结论在大多数情况下直肌止端端-端缝合术眼座暴露率更低,直肌止端端-端缝合使羟基磷灰石眼座前表面形成十字交叉结构,保护了筋膜囊与结膜,避免与粗糙的眼座表面直接接触,降低了眼座暴露率;既往眼部手术可能是眼座暴露率的另一个危险因素。
