医学研究者发现病毒性心肌炎的发病机制有以下几种。
(1)病毒直接作用：将病毒注入血液循环后可以诱发急性心肌炎，这种作用主要发生在病毒感染9天以内，人或动物的心肌中可分离出病毒，病毒荧光抗体检查结果阳性，或在电镜检查时发现病毒颗粒。病毒感染心肌细胞后产生溶细胞物质，使心肌细胞溶解，并进一步导致心肌受损。而现代的原位杂交技术显示，部分患者在病毒感染后期（9天后），尽管受感染的心肌细胞数量明显低于急性期，但每立方毫米心肌组织中仍有超过30个感染细胞，足以维持心肌炎性反应及心肌损伤。这种持续感染的病毒可能直接或间接导致心肌慢性损伤，也是慢性心肌炎向扩张性心肌病演变的重要机制之一。
(2)免疫反应介导的损伤：大多数情况下，病毒感染后期心肌内已不能再找到病毒，但心肌炎症病变仍在继续；有些患者病毒感染的其他症状轻微而心肌炎表现颇为严重；还有些患者心肌炎的症状在病毒感染其他症状出现一段时间以后才出现；有些患者的心肌中可能发现抗原抗体复合体，以上都提示免疫机制存在于心肌炎的发生发展中。参与心肌炎发病过程的免疫机制包括细胞免疫和体液免疫。其中，淋巴细胞介导的心肌损伤主要是杀伤性T淋巴细胞的激活。病毒感染诱导了CD4+T辅助细胞和自然杀伤细胞等浸润心肌，释放大量细胞因子，而这些细胞因子又诱导心肌细胞表达主要组织相容性复合体I分子和协同刺激分子等，通过一系列免疫级联反应，杀伤性T淋巴细胞前体被其识别的特异性抗原激活成具有杀伤性的T淋巴细胞，最终造成心肌细胞的损伤。而无论在病毒性心肌炎急性期和慢性期，均有B细胞的过度活化及免疫球蛋白IgG、lgA、lgM升高，提示体液免疫反应也处于增强的状态，体液免疫可能参与了病毒性心肌炎的免疫性心肌损伤。临床上，病毒性心肌炎迁延不愈者抗核抗体、抗心肌抗体、补体均较正常人的检出率为高，同样说明体液免疫参与了病毒性心肌炎的发展。所以，病毒感染后期，细胞免疫和体液免疫的共同作用导致了心肌炎的发生和发展。
(3)细胞凋亡：有研究认为，病毒性心肌炎时心肌细胞发生凋亡，产生非炎症性心肌损害，导致疾病向扩张性心肌病转化。因此认为细胞凋亡在慢性病毒性心肌炎病理损伤中，特别是向扩张性心肌病转化的过程中发挥了极为重要的作用。
